1. Did you receive "medical attention" directed by your handler or his associates over the last 72 hours for any reason?
2. Was there an injection involved?
3. Were you told that that injection was for your own good?
4. Following that injection, were you presented with paperwork from your handler or agency?
5. Was that paperwork falsely attesting that one or both of your parents had committed fraud, espionage, human trafficking, child abuse or terrorism?
6. Were you offered financial remuneration if you signed those false testaments? How much, please, and in what form?
7. Were you threatened with a lack of medical treatment for diseases caused by the FBI and its affiliates if you refused to sign the documentation provided to you?
8. Were some of the forms provided to you wrongfully alleging that you made your own decisions in fulfilling the express directives of your handlers, Txx Lxxxx, and his associates?
9. Did some of the forms provided to you wrongfully allege that you were abused by one or both of your parents during your childhood?
10. Did some of the forms provided to you wrongfully allege that one or both of your parents harmed others during your childhood or later?
11. Were you directed to release a non-conventional vector of illness within your parents' home, targeting pets or human beings, within the last 72 hours?
12. Did you do so?
13. Do you think that it may represent a conflict of interest for Txx Lxxxx and your other handlers to be directing you in these ways in an effort to exculpate themselves of their own guilt regarding wrongful directives, torture, and human trafficking toward you over a period of years?
14. How do you think this cycle of lying, false reports, coercion, monetary awards and financial theft by Txx Lxxxx and his associates can be halted decisively?
15. What can the DOJ do at this point to allow you to speak honestly and openly to your parents about the extensive harm you have experienced?
16. What can the DOJ do to ensure that your parents are involved in any and all discussions regarding your recovery?
17. What will it take for you to feel safe within the United States or elsewhere in the future?
18. What can the DOJ do to facilitate those steps on an immediate basis?

1. Have you been raped or experienced forced human trafficking by your handlers or their associates within the last 48 hours?
2. Have you experienced other forms of torture, specifically electric shock "treatment," within the last 48 hours?
3. Has your handler ever ordered you to report to another apartment or condominium within your building of residence? Have you experienced rape or torture there? How recently?
4. Has your health been harmed in any manner whatsoever by the actions of your handlers or their associates within the last several weeks? Within the last several months? What are the specific harms you have suffered?
5. Has restorative medical care been withheld from you unless you provide knowingly false witness statements regarding your own family members to the FBI, the DoD, or others?
6. Has the FBI ordered you to sign knowingly false statements, written by others, concerning your childhood, your mother, your father, or your sibling? Have others ordered you to do so? Have you been threatened with death if you elect not to do so?
7. Have employees, contractors or associates of the FBI, including the DoD, ever entered your apartment or condominium and proceeded to punch, kick, slap or beat you in order to coerce you to sign false statements regarding your own family? How recently did this occur?
8. Have you ever been told that you have received a high level order to sign false statements regarding your family members? From whom? How recently did this occur?
9. Has your handler ever told you that the United States is currently experiencing a civil war, perpetrated by the FBI and its affiliates? How recently has he done so?
10. Have you ever been told by your handler, FBI personnel, or FBI affiliates that you could be ordered to participate in a public shooting or public violence against your will? How recently were you told this?
11. Have you ever been told by your handler, FBI personnel, or FBI affiliates that your suicide would constitute a "patriotic act"?
12. Have you ever been ordered by your handler to pick up a package with contents unknown to you and then drop it off elsewhere without your ever having been made aware of the package's contents? How recently did this occur?
13. Have you ever been told by your handler that you have been tried and/or convicted of weapons trafficking or terrorism as a direct result of moving a package in that manner? How recently did this occur?
14. Have you ever been ordered by your handler, the FBI or FBI affiliates to harm your own family members? When did this occur?
15. Have you been given a deadline of December 31, 2025 by which to have perpetrated harm against your family members? Who conveyed this deadline to you?
16. Do you know whether high-level authorization was given for you to harm your family members? Do you know the name of the person who authorized this initiative?
17. Have you been ordered to place an explosive device within your parents' home or car, or in a public place with the specific intent of causing grievous injury to one or both of your parents?
18. Have you been ordered to contaminate your family members' food or drink in some manner?
19. Have you been ordered to place a radioactive device or perpetrate chemical or biological contamination at your parents' home or at their property?
20. Have you been ordered to release a "non-conventional vector" of disease at your parents' home or within their cars, intended to cause death within several weeks?
21. Have you ever been told by your handler that you have been designated as a "Code Red" by a high-level elected or appointed official within the United States' military chain of command?
22. Do you know what a Code Red is?
23. Have you ever been told by your handler or his affiliates that, after the provision of knowingly false witness statements about your family, or after the commission of harm against your family, you will be killed in order to ensure your silence regarding the torture you have experienced at the hands of the FBI and others?
24. Have you been informed that the FBI's South Bay Chief of Station has been charged with crimes against your family for his role in initiating knowingly falsified law enforcement reporting, and resultant physical harm, against your family members and yourself?
25. If this individual is persisting in attempting to wrongfully blame your family members for his own crimes, what disciplinary actions should he now face?
26. How can the U.S. Department of Justice best support you as you attempt to inform your parents of the full extent of torture you have suffered at the hands of the FBI and it affiliates?
